AKC FSS Dog Registration Application 

 

FIRST (FREE) DOG REGISTRATION APPLICATION ONLY to be sent to the Secretary by September 30, 2007 or June 30, 2008 (see SMPCA Application for details). All other applications are to be sent directly to the AKC with registration fee.

AKC Registration Requirements

The following items must accompany the completed, signed application:

 

Pedigree. Attach an 8.5” by 11” photocopy of a three-generation pedigree issued by an acceptable domestic or foreign registry. For a list of acceptable registries, see the Special Registry Services pamphlet available on our Web site: www.akc.org/registration/specreg.cfm. Do not send the original pedigree.

 

Photographs. Attach two (2) 3” x 5” close-up color photographs of the dog in a standing position: one front view; one side view. Print the dog’s name and registration number on the back of each photograph.

 

Note: If you do not have a three-generation pedigree issued by an acceptable foreign registry, you may submit export papers or any supporting documentation identifying the breed, sire, dam, and your ownership.

 

 Important notice to all Charter Members. To join the SMPCA and receive your free AKC FSS registration, you must first send your filled-out and signed SMPCA Application

 ASAP to the Secretary to record yourself as a Charter Member of the SMPCA club.

 

All Charter Members are entitled to one free dog registration. 

 

For your free registration, please complete an AKC FSS registration form with pictures and copies of the pedigree.

mail it in a separate envelope to the Club Secretary, Teresa Hickam, no later than September 30, 2007

or June 30, 2008 (see SMPCA Application for details) .
